You may also register by completing the paper form and

• Mail to: The Ho Chi Minh City Medical Association, 59B Nguyen Thi Minh Khai Str, Dist. 1, Ho Chi Minh City, Vietnam.

• Please note that a separate registration form is needed to register each participant. 

• Registrations will not be accepted by telephone.

Payment

• Payment may be made with bank transfer for faxed registration forms.

• Bank transfer information is provided in the box on the registration form.

Cancellation

• All cancellation must be in writing.

• Please fax cancellation notification to + 84-8-9301288 or email to hhyd@vnn.vn .

• Refund minus a  100USD administrative fee will be granted until 15 June, 2008.

• No refund will be possible after 15 June, 2008.
